Application Process

Application for a grant from the Sordoni Family Foundation should be made in letter form and include the following information.

1. A statment indicating the structure, purpose and history of the organization requesting the grant.
2. A brief description of the problem to be addressed, the project’s principal objectives, the proposed intervention, the anticipated results or outcome.
3. A timetable for the grant, a detailed budget, other planned sources of income and the amount requested from the Foundation.
4. Describe plans for evaluating the project’s results and for maintaining the project after the grant funds expire.
5. Submit a copy of the Internal Revenue Service ruling dated after 1969 classifying the organization as tax exempt under section 509 (a) of the Internal Revenue Code of 1954 as amended.
6. Submit a copy of the organization’s most recent Form 990.
